Attention class:
You will receive a letter(Note to Parents & a Travel Declaration Form) in your mail box within the next few days. Please complete your travel history on the Travel Declaration Form and submit to your Form Teacher on your first day of return to school in Term 3. (Soft copies of the Note to Parents & Travel Declaration Form are attached for your downloading if necessary)

If you have traveled and returned on/after 22 June 09, from the following Influenza A (H1N1) affected countries: Argentina, Australia, Canada, Chile, Dominican Republic, Hong Kong, Indonesia, Japan, Mexico, Panama, Philippines, Spain, Thailand, United Kingdom and United States of America, please do NOT report to school until after 7 days from your arrival date. (You are to rest/stay at home for 7 days after your arrival and return to school on the 8th day onwards)

Please complete and submit the Student Travel Declaration Form to the school by hand when returning to school on 29 June 2009, Monday.

Students are reminded to bring their ODTs for twice daily temperature taking when school re-opens.

Note: If you are in school and you had been to the affected countries, please proceed to the movers and shakers gallery immediately.
